Primary Care is in desperate need
of some transformation!
Total triage and patient journey – designed in house at Lincoln House Surgery just before the pandemic to help give a consistent better service without increasing the pressure on the overstretched GPs.
Reason for choosing this?
• We felt quite strongly that we needed to do something about the service we offer our patients and address the challenge of turning patients away early in the day.
• Better and more consistent patient journey
• We still wanted our workloads to be manageable
•To allow for better integration of ARRS in the appointment system
• Scope for chronic disease management
Are / were there any additional resources needed for this project?
• Workforce - Two Full Time Care Navigators (14k patient)
•Training/Mentoring/Support - 3/6momths
• Active Signposting - Designed in house with regular feedback from GP/PA/Reception Team (This will be provided as part of the package)
• IT and Premise - Yes need space and IT setup for above workforce and IT setup for GPs to work from home.
• GPs working from Home to release space in Surgery premise to allow for more ARRS staff to work.
What are the challenges/risks associated with this project?
• Different approach to managing demand in Primary Care - which is challenging for both staff and patients at the outset.
• once established patients realise - no more bottle neck
• Needs adjustments according to demand and workforce
• Daily huddle - with all GPs and ARRS along with PM helpful for learning and team building
